Short Description of Instrument |
The Diagnostic Interview for Seizure Identification (DISI) is a 16-item questionnaire that was originally developed to screen for undiagnosed seizures and epilepsy. The DISI is divided into 2 sections, with the first 9 questions to be answered by the participant and the remaining 7 questions to be answered to by an observer. Participants and observers are interviewed and asked to describe symptoms, triggers and duration of seizures. Information gathered from the DISI can then be used to make a diagnosis if applicable.

Scoring |
The DISI is a diagnostic measure and therefore does not contain formal scoring. Participants and observers are interviewed to determine seizure type, symptoms and duration.

Rationale/Justification |
This form provides screening questions to uncover seizure-like episodes in patients who may have epilepsy or seizures. Those who endorse seizure-like episodes may undergo further screening or diagnostic testing.
